Red Flag Rules

In compliance with Federal Trade Commission Red Flag Rules, Children’s Respiratory & Critical Care Specialists, P.A. has established an Identity Theft Prevention Program to detect, prevent and mitigate risks associated with identity theft. Medical identity theft is committed when someone illegally uses someone else’s personal information to obtain medical services for him/herself or others.

Effective August 1, 2009, CRCCS requests those patients (or the accompanying adult) arriving for a clinic visit present:
1. A valid form of photo identification and
2. The patient’s/parent’s insurance card (if applicable)

Valid Forms of Identification

The following are considered valid forms of forms of photo identification:
? Valid Driver’s
? License
? Valid DMV Identification Card
? Valid U.S. Military Identification Card
? Valid Passport
? Valid Permanent Resident Card (Green Card)

Should patients or the accompanying adult not possess valid photo identification, then acceptable alternative forms of identification include:
? Social Security Card
? Credit Card with Picture
? Voter Registration Card
? Certified Birth Certificate
